ORC4123.28. Record of injuries and occupational diseases; reports - Which States "The employer shall give a copy of each report to the employee it concerns or his surviving dependents."
ORC4123.75. Remedy of employee against noncomplying employer - Which States "Any employee of a self-insuring employer, in the event of the failure of his employer to pay the compensation or furnish the medical, surgical, nursing, and hospital services and attention or funeral expenses, may file his application with the commission or the bureau for the purpose of having the amount of the compensation and the medical, surgical, nursing, and hospital services and attention or funeral expenses determined; and thereupon like proceedings shall be had before the bureau and with like effect as provided in this section. "
ORC4113 MISCELLANEOUS LABOR PROVISIONS - ORC4113.21. Employee shall not be required to pay cost of medical examination. Which States " Any employer who violates this section shall forfeit not more than one hundred dollars for each violation. The bureau of workers' compensation and the public utilities commission shall enforce this section. "
ORC4113.23. Copy of medical report to be furnished to employee upon request; exception - Employer can charge twenty-five cents for a copy of report
